Forklift Truck Operator Assessment LF                  

                                                                                               

The forklift operator assessment is conducted in line with the Occupational, Health and Safety Certificates of Competency National Assessment Instrument for Forklift Trucks, August 2000.

The assessment will involve the following processes:

Administration

The applicant MUST provide the following information prior to the commencement of the assessment:

               Identification Requirements.

               Records of Training - Operator Logbook.

               Assessment Instrument.

Theory Assessment                                       

A full operator assessment consists of 52 questions and can take up to 1.5 hours to complete. To satisfy the requirements for competency the applicant must correctly answer all critical questions and a minimum of 75% of non-critical questions for each unit.

The assessment is broken up into the following units:

        *    Unit ONE consists of 20 Critical questions and 4 non-critical questions.

        *    Unit TWO consists of 9 critical questions and 4 non-critical questions.

        *    Unit THREE consists of 1 critical question and 4 non-critical questions.

Practical Assessment

Unit ONE : Conduct routine checks    

            The operator is asked to carry out a complete routine check on the forklift truck in accordance with the manufacturer's instructions.

                The operator inspects the site and plans the work. All hazards are identified where applicable.

                The operator conducts pre-operational and post start-up checks in accordance with the manufacturer's specifications/operating manual.


Unit TWO : Shifting the load

            The operator is asked to move various loads, pick up and placement of loads from low, medium and high level racking.


Unit THREE : Shut down equipment

            The operator is asked to shut down the equipment.


Unit FOUR : Secure site

            The operator is asked to secure the site.
